You do not need to be a Costco Member for an exam, however you must have a membership to purchase glasses/contact lenses.
WELLNESS EXAM $110
A wellness eye exam reviews the health of your eyes and provides an accurate prescription for glasses. Medical eye problems outside the scope of a wellness exam require a separate medical office visit.
CONTACT LENS EXAM $155 (basic), $165 (astigmatism), $175 (multifocal, monovision), $190 (standard gas permeable*****)
A contact lens exam includes a wellness eye exam (listed above) to review the health of your eyes, provide an accurate prescription for glasses and additionally, a contact lens evaluation to determine a contact lens prescription.
We fit a variety of soft contact lens modalities including daily disposables, astigmatism, multifocal and monovision.
*We do not fit gas permeable lenses for new wearers, toric and multifocal gas permeable lenses, neither do we fit hybrid lenses, scleral lenses, nor ortho-k lenses. We can refer to specialists for these advanced fittings*
All first time contact lens wearers must complete a contact lens training class ($30); which may require a separate appointment.
OPTOMAP RETINAL SCREENING $30
Our doctors recommend this optional scan for all patients, each year. An Optomap retinal scan provides a 200° scan of the eyes' retina. This provides enhanced information for the doctor to screen for glaucoma, macular degeneration, diabetic and hypertensive eye diseases, tumors, etc.
If you can not dilate your eyes, this is a great alternative.
DIABETIC EYE EXAM $190++
Diabetic Eye exams are focused on your diabetic eye health that are beyond the scope of a wellness exam.
A contact lens prescriptions can be done for an additional $30-80.
These exams are billed to Medical Insurances, not to Vision Plans.
MEDICAL OFFICE VISIT $150++
A medical office visit focuses on eye problems such as dry eyes, itchy eyes, contact related problems, red eyes, flashes, foreign bodies that are beyond the scope of a wellness exam.
We may not be able to perform a glasses/contact lens prescription exam, due to the medical visit reason.
If able to, a glasses prescription can be done for an additional $40, and contact lens prescription for an additional $30-80.
Some routine exams are converted to office visits as deemed necessary.
These exams are billed to Medical Insurances, not to Vision Plans

We can fill out many work, school, DoL forms; but not all. To confirm we can fill it out, please email it to info@sodovisioncare as soon as possible.
Most forms are covered by your exam fees, but others are subject to extra fees.
